I always take the time to read over your paperwork, begin to hold you in my thoughts and heart and formulate a plan prior to your appointment. If I do not have your information, if is more difficult to undertake this deepening of the process.

There are several options for filling out the forms below:

1) Download Adobe Acrobat here and use the “Add Text” feature to fill in the forms electronically and e-mail them back.

2) Print the Forms, write in your answers and scan to e-mail them back.

Please review and fill out my consent form prior to your appointment: FCH Client Consent 

Please download the appropriate intake form and return via e-mail within 48 hours of your scheduled appointment. Please be as open and as accurate as possible.

If you are unable to return them prior to your appointment, bringing in your completed forms to the office is the next best option. Avoid taking up to 20 minutes of your precious appointment time to hurriedly fill out forms.
